Information
We accept and
are "in-network" with most insurance plans, which is a
savings for our patients. We are also constantly adding new carriers
and our friendly, knowledgeable staff can assist you with your insurance
questions.
To check on
your specific insurance, please call or email our business office
at (301) 473-5945 between 8 a.m. and 4 p.m. Mon. – Fri. and
ask for Kelly Reichenbaugh,
Maggie Barnes or Tina
Smith.
New
Patients
Insurance
information will be verified when you schedule your first appointment.
New patients should bring the following to their appointment:
1. Prescription
from your referring physician
2. Insurance Card
3. Photo ID
Plan
to arrive 15 minutes before your scheduled time for your first appointment
so you can complete our intake forms.
Come
to your appointment suitably dressed for a mild workout. Sweats
and sneakers are perfect for a therapy visit.
All new patients
should review the Privacy Policy
forms. These outline your rights concerning your health records.
You will be required to sign an acknowledgement stating that you
have received the Notice of Privacy Practices.
Payments:
Copays are expected at time of treatment.
These are
the intake forms you will need for your first visit. If you
complete the forms before your appointment, you will save
time in the waiting room. Complete each form.
